Incident Reports

File A Report  |  View Reports

For those Veterans who, while receiving their benefits entitled to them under VA Medical Care and Treatment, have experienced any type of abuse by the system or anyone within it, we have setup our own incident bulletin board. This system is for Veterans, their families and even VA employees that have the courage to stand up against abuses they might have witnessed by providing us with first-hand accounts of the atrocities Veterans have suffered while being treated in a VA facility by VA personnel.

How does it work? Well, the first thing we do is display the report you send us on our own incident bulletin board for all other Vets and the world to see. Our guidelines page provides a detailed outline of the information that we feel others most need to hear. As much detail as you give us, we, in turn, give to others. You tell us, and we tell the world.

We will never identify you by name or number, and any personal information you give us about yourself will remain forever CONFIDENTIAL. However, we will identify all staff members or facilities by name if you provide us that information. We will let the world know what you have told us.

Then, after verifying that, in fact, you are a real person (and that is the only criteria we consider), we forward the report to various responsible parties within the government. For instance, the Senatorial sub-committee for Veterans Affairs, the VA Inspector Generals' Office, the Director of the VA himself, and any other parties of which we know will be forced to take some action once they know that the report has been made public. And made public it will be!

The only thing we need is YOU and your fingertips. We need you to tell us by filing an incident report here online, and we need you to tell your friends and other Veterans what we are doing and how to find us. The VA would never tell you about us and they would love to shut us down. They cannot.

We will do this and we will continue to do this and other things until the abuse that Veterans have long suffered at the hands of the system ends.
