Guidelines

You MUST identify yourself to us ONLY and provide us with a valid email address, or we will not publish your report. We may need to prove some day that we did not make up the report just for the heck of things.

You are NOT allowed to use any foul language.

You should provide us the following information:

  1. The location of the incident (Required)
  2. The date of the incident (Required)
  3. The nature of the incident
  4. Are you a witness to the incident or the individual involved? (Required)
  5. The names (as exact as possible) of the staff members involved
  6. The positions of those staff members, if known
  7. The service being performed or not performed as the case may be
  8. The nature of the abuse
  9. The complications caused by the abuse
  10. The damages, if any, incurred by the abuse
  11. The description of the incident and, in your own words, what happened (Required)
  12. Forward this report for corrective action

If, instead of filing an incident report, you wish to submit a long horror story (and there are many, many, many), you need to do so in a free-form email to our email address. As we update our site periodically, we will pick out those stories that are the real thing and that would be of interest to other Veterans and the general public, and we will display them in a separate area for all to read. We will also forward your story to the appropriate channels unless you ask us not to do so.

Remember, once posted, the report will not be removed unless requested by the person who filed the report, so it is best to identify yourselves to us for at least that reason.

Look, we all know that there are some really great people working in the system on our behalves with our well being utmost in their minds. Do NOT use this site to send kudos to any person or facility. Send all kudos directly to those people or to the director of their facility. This is anything but a kudos site.

WE DO NOT INVESTIGATE REPORTS. WE RELY ON YOU FOR HONESTY. WE ARE NOT RESPONSIBLE FOR THE CONTENT OF THE REPORTS WE PUBLISH. WE DO NOT GUARANTEE THEIR AUTHENTICITY. THE GOVERNMENT AGENCIES TO WHOM WE FORWARD REPORTS ARE RESPONSIBLE FOR THEIR OWN INVESTIGATIONS. THAT IS WHY WE PAY TAXES. THAT IS WHY THEY GET A PAYCHECK AND WE DO NOT.

If it is not factual or not true, do not file a report. This site is not meant to hurt anyone. It is meant only to HELP Veterans and their families.
